Hello Brentt,
Thanks for your question. Whenever a device is factory reset it is restored to the version it was as a new product. As a result any changes made or apps installed would need to be redone or reinstalled. This includes Covenant Eyes. You will want to reinstall Covenant Eyes fresh from the App store or Play store, sign in with your Covenant Eyes username and password, and then follow the appropriate setup steps.
